I’ve read the Privacy Notice and know how my personal data will be held and used. I’ve read the Prevention of Money Laundering Statement and acknowledge that Aviva may verify my identity and address using a third party company. I’ve read about my rights under the Access to Medical Reports Act 1988. I know that Aviva might need to seek medical information about me as part of my application or for audit purposes. I also know Aviva may need to seek medical information about me in the event of a claim. The answers I have provided about my Health & Lifestyle are truthful, accurate and complete. I’m aware that if any of the information I have provided to Aviva is not truthful, accurate or complete and might reasonably have affected their decision to provide cover, then Aviva may amend or cancel the policy, may reduce the amount payable in the event of a claim, or may not pay a claim. I am aware of the importance of reading the Policy Summary and Policy Conditions and that these documents, together with the Policy Schedule, this Declaration and any information given in the application process will apply to my policy. If it is necessary to assess my application, I’m aware that cover will not start until Aviva has accepted my application, received a completed Direct Debit mandate, and confirmed when cover will start. Further, I am aware that I need to let Aviva know if any of the answers to the Health & Lifestyle questions change before they confirm when cover will start, and that any changes may result in alterations to the cover, premiums or benefits offered. I am resident in the UK, and have permission to legally remain in the UK on a permanent basis.
